I have Lost Access to My Duo Device

If you have lost your phone or can no longer access the method you setup to for two-factor authentication, and you cannot login, you must call the UFIT Help Desk at 352-392-4357 so that they can verify your identity and help you add a new device.  

In order to verify your identity, the UFIT Help Desk will need to verify a government issued ID by video conference or in person.
